Health Journey Map
Facilitating a fusion session to journey map the full patient experience—from checking into a doctor's office through diagnosis, care planning, and follow-up with an electronic buddy system.

Overview
The Health Journey Map was designed based off of a fusion session in which a team of UX, UI, and subject matter experts went through a card sort to try and alleviate pain points to which patients may experience while seeking medical treatment.
The Process
The journey defines what the patient may go through from checking into the doctor's office, consulting with their physicians, diagnosing and coming up with a care plan, and setting up follow-up actions with an electronic buddy system.
The Output
The map was the resource guide to which we utilized as our guiding map when working on the project.
